This usually occurs when you don't pay attention to the headers, in
particular stuff in the >Cc: field. You might be replying to a message
from the list, fix up the >To: field but not clear the >Cc: field -- if
beam@whatever is in both places, you'll get two copies (and so will
everyone else).
If your own address is in the >Cc: and you send to the list, you are the
only one who gets the duplicate.
There are some other mechanisms, but I don't think the list is broken.
